Telehealth consent

All services at this practice are provided by telehealth — the use of electronic communication technologies (live video, audio-only telephone when clinically appropriate and permitted by law, secure messaging, and electronic records transmission) to deliver care remotely, including psychiatric evaluations, medication management, follow-up visits, care coordination, and patient education. Telehealth may not be appropriate for every condition; the provider may recommend in-person care or a referral when clinically indicated.

Benefits and limitations. Telehealth improves access, convenience, and continuity of care — and it has limits: a full physical examination is not possible, non-verbal cues are harder to observe, and sessions depend on technology and connectivity. If technical issues occur, the provider will attempt to reconnect, use an alternative contact method, or reschedule.

Privacy and your responsibilities. The Practice uses reasonable safeguards consistent with HIPAA standards, including secure platforms and access controls; no system is completely secure, and privacy cannot be absolutely guaranteed. Patients agree to join sessions from a private location on a personal, password-protected device, to avoid unsecured networks when possible, and not to record sessions without prior written consent; sessions are not recorded by the Practice without explicit consent. Confidentiality may be limited where there is risk of harm to self or others, suspected abuse or neglect, or disclosure required by law.

Your rights. You may refuse or discontinue telehealth at any time, ask questions about risks, benefits, and alternatives, and withdraw consent without affecting your right to future care. Your identity and physical location are confirmed at each session. In both California and Oregon, telehealth consent is obtained and documented, you are informed of the risks, benefits, and alternatives, and telehealth care meets the same standard of care as in-person treatment.

Emergencies. Telehealth is not appropriate for emergencies. For suicidal or homicidal thoughts, severe psychiatric distress, medication reactions, or any urgent medical issue: call 911, go to the nearest emergency department, or call or text 988.

The full Telehealth Informed Consent Agreement is reviewed and signed as part of intake paperwork.
